1. Fill out the form accurately. All details provided by the student will reflect on Filamer's Centralized System (FilCIS) during the enrollment.
2. For applicants of College of Nursing, be advised that the student needs to take the Nursing Aptitude Test (NAT) before filling out the form. Kindly visit the Guidance and Counseling Center at the Main Campus for more details regarding the said test.
3. After filling out the form, the student can proceed to the Annex Campus (Technology Building) Room 201 for the Entrance Exam.

Entrance Exam is administered every Saturday (except holidays). Details regarding the entrance examination are provided at the last part of the form.
